Information required for verification:

1. A recent head shot of your athlete with no objects in the background and no hat or sunglasses on.

2. Identity document(s) - birth certificate or passport are acceptable

3. School document - most recent report card with all information listed (Athlete’s name, School name, Grade, etc.)

4. Sign Naugatuck waiver - electronically sign

5. Payment - there is a $5 fee for processing verification, provide payment info here (please note there are advertisements on this page that need to be marked either “yes” or “no” otherwise it will not let you proceed)

6. Medical Clearance Form

7. 2024-2025 Final Report Card (this can be added at a later date)

All information must be uploaded and completed by July 27, 2025.

Once those forms are received, National Sports ID will take the necessary steps required to verify your athlete and 

you will be required to review and complete the AYF/AYC player packet

(this area will require insurance and doctor information).
